Bosentan lowers blood pressure in your lungs, helping your heart pump blood more efficiently. Bosentan is used to treat pulmonary arterial hypertension (PAH). It improves your ability to exercise and prevents your condition from getting worse. Do not use bosentan if you are pregnant. You will need to have a negative pregnancy test before starting this treatment. You must use two highly effective birth control methods to prevent pregnancy while you are using this medication and for at least 30 days after your treatment ends. To make sure bosentan is safe for you, tell your doctor if you have liver disease diabetes an autoimmune disorder such as rheumatoid arthritis or psoriasis or if you have had or will have a heart or kidney transplant.
Typical Uses
Bosentan is used to treat high blood pressure in the lungs (pulmonary arterial hypertension). This condition is thought to be caused by increased levels of a certain natural substance (endothelin-1). This medication blocks the effects of endothelin-1, thereby helping to decrease the blood pressure in the lungs, slow the worsening of symptoms from the disease, and improve your ability to exercise.
Side Effects
Get immediate medical assistance if you have signs of liver problems- nausea, upper stomach pain, itching, tired feeling, loss of appetite, dark urine, clay-coloured stools, and jaundice (yellowing of the skin or eyes). Contact your healthcare professional if you experience swelling in your legs or ankles, with or without weight gain pale skin, feeling short of breath, rapid or irregular heart rate, trouble concentrating a light-headed feeling, like you might pass out or new lung problems-anxiety, sweating, pale skin, severe shortness of breath, wheezing, gasping for breath, cough with foamy mucus, chest pain, fast or uneven heart rate.
Drug Interactions
• Bosentan+hydrocodone- Bosentan may reduce the blood levels of hydrocodone, which may make the medication less effective in treating your pain. Additionally, if you have been receiving treatment with hydrocodone, adding bosentan may cause you to experience unpleasant withdrawal symptoms such as watery eyes, runny nose, sneezing, yawning, excessive sweating, goose bumps, fever, chills, flushing, restlessness, irritability, anxiety, depression, pupil dilation, tremor, rapid heartbeat, body aches, involuntary twitching and kicking, abdominal cramping, loss of appetite, nausea, vomiting, diarrhoea, and weight loss.
• Bosentan+bosutinib- Bosentan can significantly reduce the blood levels of bosutinib, which may make the medication less effective in treating your condition. Talk to your doctor if you have any questions or concerns.
• Bosentan+cobicistat- Talk to your doctor before using bosentan together with cobicistat. Combining these medications may significantly increase the blood levels of bosentan, potentially increasing the risk of side effects. You may need a dose adjustment if you have been receiving bosentan and are starting treatment with cobicistat, or vice versa.
• Bosentan+daclatasvir- Bosentan may significantly decrease the blood levels of daclatasvir, which may reduce its effectiveness in treating hepatitis C. Talk to your doctor if you have any questions or concerns.
Mechanism of action
Bosentan belongs to the class of medicines called endothelin receptor antagonists. It works by stopping the action of a naturally occurring protein called endothelin-1 that causes blood vessels to narrow and prevents normal blood flow in people who have pulmonary arterial hypertension.
